Once completed the form should be returned together with a copy of any relevant baptismal/dedication certificate to:
St Anne's Catholic Primary School
Please note emailed supplementary forms are not admissible, due to the fact that parental signatures are required.
Supplementary forms can be completed for a child of any age and returned to school. Once received forms are placed on file and the Local Authoity pack will then be sent to you at the appropriate time.
Parents of baptised Catholic children, or children dedicated in another Christian denomination, are required to give full details of baptism/dedication and provide a copy of the baptismal certificate with the supplimentary form. The school requires copies of your child's birth certificate prior to their entry.
Applications received after the closing date will be accepted but considered after those received prior to the closing date. You are therefore encouraged to ensure your application is received on time.
Applications from parents made during the school year, outside the normal round of admissions, should be submitted to the Local Authority in the same way as above.
